Dr. Fordham is a Colorado native who received all of his education at Colorado State University. He earned a bachelor's degree in Animal Science in 1999, a master's degree in Anatomy in 2000, and his DVM degree in 2004. He has practiced at Mountainview Animal Hospital since 2005 and shares his practice with Dr. Monica Hazelwood and Dr. Jeff Longely. Dr. Fordham enjoys internal medicince, geriatric medicine and the human animal bond. Outside of the office, Dr. Fordham and his lovely wife, Shawny, have two young children, Reece and Joelle who keep them on their toes. They also have two active dogs, Bailey a black lab and Brady, a husky mix. Dr. Fordham is an avid baseball and football fan, loves to cook and enjoys real estate.
Education:
Dr. Hazelwood was born in Brooklyn, New York, but grew up in Phoenix, Arizona. She attended Colorado State University for both undergraduate work and to earn her Doctor of Veterinary Medicine degree. In veterinary school her nickname was "Doogie" because she was the youngest member of her veterinary class. Upon graduation, she worked in Emergency for several years at the largest emergency practice in Phoenix, Arizona, and then later switched to daytime practice because she loved building relationships with pets and their owners. Professionally, Dr. Hazelwood enjoys internal medicine with an emphasis on diagnostic interpretation, feline medicine especially with geriatric cats, and dentistry. She loves to mentor and teach. Recently, she started teaching a 2 hour Pet First Aid and CPR course to various groups in the Denver area. Dr Hazelwood lives with her husband, Scott, precious baby girl Abigail, her German Shepherd, Pete, her Rottweiler, Ellie Maye and her two cats, Emmie and Scout. She enjoys hiking, country and salsa dancing, and snowboarding (which she is still trying to learn).
Dr Longley, pictured here with his buddy Garmin, grew up on the east coast of Canada in Halifax, Nova Scotia before heading to Kansas for college. He graduated from Kansas State College of Veterinary Medicine in 1992. Aside from short stints in Las Vegas and Seattle, he has spent the majority of the past 20 years in small animal practices in the Denver Metropolitan area. Dr Longley's special interests include feline medicine, ultrasound and preventative care. Dr Longley emphasizes the importance of communicating with pet parents to ensure they have the information, resources and knowledge to provide proper nutrition and optimal care to their favorite companions. His hope is that this crucial information will provide each client the best chance that their companions will live long, happy and healthy lives. Dr Longley, his wife Amy and their daughter Megan share their home with several beloved critters: Puck, a diabetic Maine Coon kitty; Abie, a golden retriever rescued after six years as a breeding dog in a puppy mill; Garmin, a lab/cattle dog mix misplaced by tornados in Oklahoma; and Mater, a maltese/Jack Russell terrier mix who was another rescue that his wife thought was just too cute to walk away from. An avid cyclist, Dr Longley enjoys participating in many events here in Colorado such as the Triple Bypass, and the Children's Hospital Courage Classic.
